There are two necessary steps in completing your  Application to The Hatlen Center:

1. Download and print the PDF (you need Adobe Reader) file below, complete, sign, and mail these forms to:

    Release of Information Forms in PDF Format

If you cannot open the PDF document, please call us at (510) 234-4984 and we will mail it to you.

2. Fill out an application.  The application is available in two formats - as a PDF file or as a Microsoft Word document.  The PDF file (requires Adobe) can be filled out and submitted online, and the Microsoft Word document can be printed, filled out, and mailed to us at:

    **Make sure your left and right margins are set to 0.5 inches before printing.
