There are two necessary steps in completing your Living Skills Center Application:

1. Download and print the file below, complete, sign, and mail these forms to the Living Skills Center

    Release of Information Forms in Microsoft Word Format

    Release of Information Forms in Generic Text Format

2. Fill out an application.  The application is available in two formats - as a PDF file or as a Microsoft Word document.  The PDF file (requires Adobe 7.0) can be filled out and submitted online, and the Microsoft Word document can be printed, filled out, and mailed to us at:
